SETUP
In addition to the electronic management agreement, please download and complete the W-9, a Direct Deposit Authorization Form, and Insurance Authorization form. All of these documents once completed are very sensitive. Please either fax or mail to Welcome Home.
If you are not a US Citizen, please complete a W-8ECI Form. If you do not have a US Tax ID, you can arrange to obtain one from Richard Hart.
